Died on the moon right next to my respawn point, no gravestone appeared. Subsequent deaths spawned gravestones no issue.
OK to sum up here as well as in the ticket… We dug around on the moon and we looked around with the logs and were unable to find anything more. Given that the current grave mod on SevTech is simpletomb which doesnt seem to have the grave commands advertised, we are drawing a blank and as we walked about on the server, we are more or less calling it.
