Self infusions are a thing in thaumic horizons to give players buffs and powers, I've given myself 4 infusions: awakened blood, diamondflesh, warp tumor, and crafting fingers. Normally, they would only go away if you die. I've given myself infusions twice, and last time, one by one, they all disappeared for no reason. Some were buffs, like diamondflesh(resistance) and awakened blood(regen), others were powers like climbing on walls, or breathing underwater, or having less warp. This time, I got warped tumor, diamondflesh, awakened blood, and morphing fingers, which is a portable arcane crafting table you activate by hitting the key it's bound to. With crafting fingers, I've noticed that if I go to another dimension, I've had to relog for it to work. After going to the nether and coming back through a normal portal, it has stopped working completely. Now I think going to another dimension might result in me losing my other buffs. The time when I lost it I didn't relog upon going in. There is an item in creative to give yourself an infusion to test this with.
Normal buffs also appear to get rid of self infusions as well, normally they are supposed to go over the infusion, and then the infusion's buff will come back. I just lost regen... again.
KCauldron is almost certainly causing your issues with loss of the buffs on dimension change, as I would assume infusions work similarly to AM2 magic level, which gets messed up when this occurs. Just to confirm, do you have the AM2 Dispelling Imbuement on your leggings?
Can you reproduce the issue in singleplayer? Does it only happen if you switch dimension? Whats the name of the creative item?

It appears to be okay without the pants on. The creative item is just Self: and the infusion's name, for testing purposes the most easily tested are the buffs and morphic fingers, since all you need to test the fingers is to hit the key to activate them.
I've tested it in single player and it seems that when going to another dimension with dispelling on your pants it removes the effect but without them it doesn't so it isn't server side. For the morphic fingers that IS server side cause it seems to work fine for me Quick edit with dispelling on your pants in single player when you recognized the effects come back :/
