Done glass blocks on heads

i keep seeing glass blocks show up on mine, players and mobs heads and can not get them to go away

This looks like the galacticraft oxygen mask. Are you by chance wearing one? If so, that would cause it to appear for you. As for mobs, I have no idea what would cause that. Please let us know!

I am not, I log out and it gets rid of it for a bit but it comes back

 

Can you try and reinstall the pack for me?

 

Ok i reinstalled the pack of still happens after awhile I will let you know
