Implemented Error when I try to go to the end

    I keep getting the following error when I try to go to the end via my portal.

    I had been in the end killing the endermen for the ender shards and went back to my base to recharge my wand and replace my broken sword and now it does this when I try to got back. 2015-03-18_16.43.19.
