Done graves not always appearing

so I have died 2x without a grave appearing now or items dropping and im wondering if there is a way to fix this bug IK i wont get my items back but it would be nice if it doesnt happen again

 

Have you look around the area you died? Sometimes the grave spawns at weird positions.
Had this several times, grave spawned like 20~ blocks away

 

I checked within the supposed 5x5 radius they can spawn in
