I am confused by your question. I just drop the jar file into each mod folder for the pack I want optifine installed in.

The ban is tied to your Microsoft account, so you would need to set up an entire new Microsoft account and then buy a new Minecraft account for it.
Server owners will have the option to have it be turned on or off. I hope that MyM decides to keep it turned off.
