most of the time you can fix this by changing what version of optifine you have installed or by changing some things in optionsof.txt after the...
next time post a crash report intead so we and staff can find what was the cause :)
it might also of dropped on the ground if you were using a me terminal, tends to happen from time to time if the server lags while you pull stuff out

you could hide the items somewhere(nether aether etc) and come back for them when you can, just make sure to get them before that dimention resets.
What version of the modpack do you have installed? mym is running 1.2.0 on their servers atm.
