What are the /tps while you had that issue?
Changed Sky Farm - Irrigation quest to the correct items
Removed the achievement and added the staff to your inventory. I also added a warning.
Fixed all titles were unlocked for all tiers
Is there a reason why this is marked as done? I can remove the achievement and give you the staff of power back if you want

Do you have the issue on a places on the server? (your island, spawn..) What about singleplayer? Do you have fastcraft installed?
Reduced Veinminer limits from 10000 to the default value of 800 (How did the modpack dev think that would be a good idea..?) Edit: Also changed...

Did you use any item to get that to spawn or did you just teleport into that world?
When does it spawn? In the worldgen? Does it show up in singleplayer?

Hm, no idea then. Do some other quests, I don't think you would get a required item from that quest.
There is no easy way to find out why it is not working and it is also no important issue. Marking as done.
Server is fine again, marking as solved.
