It would be useful to see the client log from around the time of the second issue occurring, though. Do you use Optifine/Shaders in this pack?
This isn't actually a crash report, just your client log. Crash reports can be found in the folder labelled "Crashreports" in your install folder....
I know there used to be issues with Soulbound in other dimensions. Might have been AM2's one only.
